MIPS Proposed Rule 2022: MIPS Value Pathways (MVPS)

The MIPS Value Pathways maps out the framework to connect all four MIPS performance categories (Quality, Cost, Improvement Activities and Promoting Interoperability) and an additional Population Health segment, into the goal of improving performance measurements. Each MVP contains activities and measures that will tie into a specialty, medical condition, or episode of care, which QPP will use to provide more meaningful feedback to clinicians and more value to patients. CMS is proposing the transition to MVPs will be complete by 2028.

In the 2023 performance year, CMS is proposing to offer seven MVPs to include specific measures in the Quality, Improvement Activities and Cost categories, which together make up the first layer of the MVP framework. The second layer will contain the Promoting Interoperability category, structured similar to how it is today, and Population Health measures, which will address hospitalizations and readmissions within MVPs.  The proposed MVPs include:

  • Rheumatology
  • Stroke Care and Prevention
  • Heart Disease
  • Chronic Disease Management
  • Emergency Medicine
  • Lower Extremity Joint Repair
  • Anesthesia

It should be noted that at this time there are no clinical MVPs proposed for Urology or Oncology specialties, but CMS is expected to prepare new MVPs for additional specialties in the future.

CMS is proposing voluntary participation in the MVPs. Practices who want to make the transition to MVPs in 2023 will have to register between April 1, 2023, and November 30, 2023 to participate, including choosing the MVP that the practice wishes to report.

Eventually MVPs will become mandatory for practices as the traditional MIPS program is expected to be discontinue after the 2027 performance year. CMS plans to continue to develop and implement new MVPs during this transitional timeframe. They are hoping by offering these programs in parallel, practices will transition from traditional MIPS into a specific MVP once one becomes available.  CMS is encouraging providers to submit feedback on the proposed timeline.
